Dwarfs are random mutations. A lioness can randomly have one, and mutation items like cotton root bark, lion meat, and GMO cow boost the chance of the random mutation rolling. A dwarf is a very rare random mutation though, and you're probably better off trying to buy one than hope for it to randomly appear.

Again, the mutation items boost the random chance, not the rarity of the resulting mutation.

As an additional note, you need a dwarf mother for the lion scrotum to potentially work. If it's just a non-dwarf lioness, you just need insane luck, even with boosting items.
